BACKGROUND: Various approaches using epidural analgesia have been employed for relieving labor pain and promoting spontaneous delivery. We aimed to evaluate the effect of nalbuphine and ropivacaine versus fentanyl and ropivacaine on the duration of delivery in parturients. METHODS: Clinical data of 160 full-term primiparous women who received either nalbuphine or fentanyl in combination with ropivacaine infusion for epidural labor analgesia in our hospital from December 2020 to May 2022 were retrospectively analyzed. The participants were divided into two groups based on anesthesia methods: nalbuphine group (NR group, n = 78) received 0.2 mg/mL nalbuphine combined with 0.1% ropivacaine hydrochloride for patient-controlled epidural analgesia (PCEA) and fentanyl group (FR group, n = 82) received 2 ug/mL fentanyl citrate and 0.1% ropivacaine hydrochloride for PCEA. Both groups received an epidural blockade for labor analgesia at lumbar 2-3 interspace. The duration of the first, second, and third stages of labor, the onset of analgesia, and time before delivery (T0), 15 min of analgesia (T1), 30 min of analgesia (T2), full opening of the uterine opening (T3),exerts force during childbirth(T4), heart rate (HR), blood pressure (BP), blood saturation (SpO2), visual analogue pain scale (VAS) score, Ramsay sedation score, and modified Bromage score, and 5 min were recorded at 2 h postpartum (T5). The neonatal Apgar score, neonatal behavioral neurological assessment (NBNA) score, maternal nausea, vomiting, and itchy skin were recorded. RESULTS: Compared with the FR group, the first stage of labor duration (p < 0.05) and total duration of labor (p < 0.05) were shortened and the onset of analgesia (p < 0.05) was increased in the NR group. NR group had lower incidence of urinary retention than FR group (p < 0.05). The maternal and neonatal investigational parameters and scores had no significant difference between the two groups. CONCLUSIONS: Nalbuphine combined with ropivacaine in epidural block labor has a faster onset of analgesia and has a lower incidence of urinary retention than fentanyl combined with ropivacaine, and nalbuphine shortens the duration of the first and total stages of labor. Both nalbuphine and fentanyl can reduce pain during labor, have little effect on maternal hemodynamics, and have no significant effect on neonatal Apgar or NBNA scores.

OBJECTIVE: To analyze the fat and fatty acid composition of Fujian characteristic snacks, and to evaluate their nutritional value. METHODS: The fat and fatty acid composition of Fujian characteristic snacks were analyzed by the national standards, and analyzed by statistics such as correlation analysis, nonparametric test and cluster analysis. The nutritional value of fatty acid was evaluated by unsaturated index(UI), atherogenic index(AI), thrombosis index(TI). RESULTS: The contents of crude fat and total fatty acid were 0.11-26.35 g/100 g and 0.08-17.85 g/100 g. The correlation coefficient r=0.915(P<0.01), crude fat and total fatty acid in poultry and livestock meats were higher than those in seafood(P<0.05). The main saturated fatty acids(SFA) were palmitic acid(21.80%-34.00%)and stearic acid(7.72%-21.45%), and the main unsaturated fatty acids(UFA)were oleic acid(5.38%-41.17%) and linoleic acid(2.99%-23.54%). The content of eicosapentaenoic acid(EPA) in fish noodle(0.71%) and docosahexaenoic acid(DHA) in fried fish roll, fish slide, fish noodles(0.41%, 2.12%, 1.59%)were far higher than other snacks. Polyunsaturated fatty acids/saturated fatty acids(PUFA/SFA)ratios were 0.10-0.69, there were 8 kinds of snacks with PUFA/SFA ratio value>0.4. The n-3 PUFA/n-6 PUFA ratios were 0.01-0.75, n-6 PUFA/n-3 PUFA ratios were 1.33-74.31. The n-6 PUFA/n-3 PUFA ratios of fish slide(1.33) and fish noodle(2.13) conform to the ideal ratio stipulated by the British Health Organization, and the ratio of flat food(6.36) conform to the recommended ratio of Food and Agriculture Organization. The SFA∶monounsaturated fatty acids(MUFA)∶PUFA ratios were 0.8∶1.0∶0.5-2.4∶1.0∶0.2, the ratios of pork zongzi, meat yan silk, spicy meat, pressed salted duck and fried oyster(1.2∶1.1∶0.5, 0.8∶1.0∶0.6, 1.0∶1.0∶0.7, 0.8∶1.0∶0.5 and 1.1∶1.1∶0.5) were close to 1∶1∶1. The UI of 15 snacks was 48.37-90.67. The AI was 0.40-1.44, and the TI was 0.97-2.32. When the distance scale was 9, snacks were divided into three categories, namely fish noodle, fish slide and others. Fish noodle was high EPA and DHA type, and fish slide was high DHA type. CONCLUSION: The fat and total fatty acid contents of the 15 snacks were wide, which were closely related to the raw materials and cooking style. Nutrition and health value of fish slide, fish noodles food, meat yan silk and spicy meat were high.

Polyethylene terephthalate (PET) is one of the most commonly used substrate materials in the field of flexible electronics, and its stress-induced birefringence often has a detrimental effect on the optical properties of the device. Therefore, a deep and systematic understanding of the stress-optical properties of PET films is crucial for device design and manufacture. The photoelastic method is a direct optical measurement technique based on the stress-induced birefringence effect of materials, which has the advantages of being nondestructive and noncontact. In this work, the photoelastic method was used to quantitatively characterize the anisotropy of the stress-optical properties of PET films under the uniaxial stress state. First, a self-built reflection-transmission coaxial bidirectional photoelasticity measurement system was developed by means of a combination of transmission and reflection photoelasticity. Then, the stress-optical coefficients and isoclinic angles of PET films with different stretching angles were measured. Finally, the linear combinations of the photoelastic tensor components and refractive-index-related parameters were determined by fitting the analytical relationship between the stress-optical coefficients and isoclinic angles.

BACKGROUND: As a developing country with the world's largest population in a state of economic transition, reforms to China's health system, including community health services (CHS), are very complex and difficult. The aim of this study is to provide evidence and policy recommendations for the sustainable development of CHS for China, which could also be applicable to other developing countries. METHODS: A cross-sectional survey was conducted door-to-door and face-to-face in Wuhan city, central China with a sample of 1134 individuals aged 15 and older. The independent variables were duration of residence, previous treatment experience, familiarity with health staff, self-reported family economic status and health insurance. The dependent variables were views on the usefulness of CHS and willingness to use them. Sociodemographic variables and health status were used as control variables. Multiple logistic regression analysis was used to analyse the influence of the independent variables on the dependent variables. FINDINGS: This study shows that 26.10% of participants reported that the CHSs are not useful and 37.74% reported they did not want to use their CHS. The results found 'familiarity with health staff' and 'previous experience of using services' had a negative influence on their views on usefulness of and willingness to use CHS. CONCLUSION: The aim of CHS to see 'minor illnesses treated in the community and serious illness treated in hospital' is not being fully realized. The key to increasing the use of CHS may be to enhance the quality of services and health staff. A policy pathway of targeting older residents and those with higher education levels as the priority population, and using these groups to encourage the rest of the community to seek minor services at CHS, may be an effective and sustainable development mechanism.

BACKGROUND: The family, together with the individual and society, and the state and market are the three pillars of well-being and social security over the life course, with responsibility for the health of older adults. OBJECTIVE: The aim of current study was to explore the effect of intergenerational and intragenerational support on perceived health of older adults. METHODS: A cross-sectional survey with 64.36% (1318/2048) response rate was conducted door to door in rural China with a sample of 1318 individuals aged 60 years and older. There were three sets of independent variables: intergenerational support from children as well as intragenerational spousal support and intragenerational sibling support, sociodemographic variables, and behaviour variables. Multiple logistic regression analysis was used to analyse the influence of family member support on physical and psychological health. RESULTS: After adjusting for potential confounding variables (sociodemographic variables and behaviour variables), it was observed that intergenerational support from children had a significant effect on the self-rated physical health and psychological health of older adults [odds ratios (OR) = 1.48, 95% confidence interval (CI): 1.10-2.00; OR = 1.82, 95% CI: 1.36-2.44, respectively]. However, intragenerational support from spouse and siblings was not significant. CONCLUSION: The importance of intergenerational support from children would suggest that family practice and health-related policy of older adults may need to be extended to their children. With the rapid development of global population movements, family practices need to pay attention to the changes of family support among migrant families.
